catch record

i am under 15 and need a catch record card for steelhead i am visting drom indiana what will i need to give them to get one? do i need my birth certificate?
Since you are 15 you will have to have but not buy a license.....so just ask for a catch card as well. You will need to to provide your social security number.
Does that make sense?
